We work with Tricare, Colorado Medicaid, CHP+, and most private insurances.
To find out if we accept your insurance provider, please contact our clinic directly.
In order to bill insurance, we will need a referral/prescription from the physician.
We do not require the Rx/referral to set up an evaluation, but it is helpful to have it prior to the appointment. It can slow down the timeline to fitting if we don't have it up front.
There is no charge for a consultation.
